Notice of Privacy Policy
and Practice

Purpose of this Notice: Our pharmacy respects the privacy of personal information and understands the importance of keeping this information confidential and secure.  This Notice describes how we protect the confidentiality of the personal information we receive.  Our practices apply to current and former employees and customers.

Types of Personal Information We Collect: As a community pharmacy we collect the minimum personal information required to provide our patients with the best pharmacy services possible.  To fill your prescription, we collect the following information: patient name, address, date of birth, gender, drug allergies, medical conditions, illnesses, and insurance information which may include the patient’s or a family member’s social security number.

How We Protect Personal Information: We treat personal information securely and confidentially.  We limit access to personal information to only those persons who need to know that information to provide support services to our patients.  These persons are trained on the importance of safeguarding this information and must comply with our procedures and applicable laws.  We meet strict physical, electronic and procedural security standards to protect personal information and maintain internal procedures to promote the integrity and accuracy of that information. All personal information is kept on site and our computer system is for this store only. Information in our computer system is not accessible to any other pharmacy site.

Disclosure of Personal Information: We may share any of the personal information we collect (as described above) with our affiliates as permitted by law in the normal course of filling your prescription.  We may also disclose this information to non-affiliated entities or individuals as permitted by law.  Persons with whom we may disclose information as permitted by law include our attorneys, accountants and auditors, a customer’s authorized representative, healthcare providers, prescription benefit managers, law enforcement and through court subpoena.

Individual Rights to Access and Correct Personal Information: We have procedures for an authorized representative of the information to have access to the personal information that we collect, and other than information we collect in connection with, or in anticipation of, a lawsuit or legal claim, we will make this information available to the user upon written request. We have procedures in place to ensure the integrity of our information and for the timely correction of incorrect information. You may request a copy of your pharmacy profile and original prescriptions for you to inspect for accuracy and to request changes. This should be done at the pharmacy where you most recently received a prescription. You have the right to request a copy of our Privacy Policy every three years from the date of your most recent prescription. To do so, contact us at the pharmacy where you most recently received a prescription. If you feel your rights have been violated, it is within your rights to report this to the Secretary of DHHS. This must be reported within 180 days of when you feel your rights were violated.
